About Vitali-T

Vitali-T Health & Wellness Clinic is a clinician-led hormone optimization and performance medicine practice based in Astoria, Oregon. We currently serve eligible patients in Oregon and Washington through a hybrid model of telemedicine and in-person care. Our services focus on men’s and women’s hormone optimization, medical weight management, metabolic health, sexual wellness, body composition, recovery, and selected performance medicine therapies when clinically appropriate. Vitali-T plans to expand through a phased state-by-state model, beginning with additional western states and gradually adding more states across the United States as practitioner licensure, prescribing authority, pharmacy access, and clinical infrastructure are established.

Vitali-T provides individualized, laboratory-guided medical care rather than one-size-fits-all treatment. Treatment decisions may consider:

How care works

Patients complete a medical history, symptom questionnaires, required consents, and baseline laboratory testing before or as part of their evaluation. Laboratory testing may be completed at the Vitali-T laboratory in Astoria, through approved home testing, or through outside laboratories such as Quest or Labcorp when appropriate. Clients cannot use their own insurance. Patients then meet with a licensed medical practitioner who reviews symptoms, history, laboratory findings, treatment goals, risks, contraindications, fertility considerations, and available treatment options. When treatment is medically appropriate, an individualized plan is created. Patients receive ongoing laboratory monitoring, clinical follow-up, medication management, and education.

Men’s hormone optimization

Vitali-T evaluates men with symptoms that may be associated with testosterone deficiency or other hormonal or metabolic conditions. Evaluation may include:

• Total testosterone
• Free testosterone
• SHBG
• Albumin
• LH and FSH
• Estradiol when appropriate
• Hemoglobin and hematocrit
• PSA when clinically indicated
• Blood pressure - self- or primary care reported
• Thyroid and metabolic markers
• Fertility goals We do not evaluate testosterone solely by looking at one total testosterone result. Total testosterone, free testosterone, SHBG, laboratory timing, symptoms, treatment response, and safety markers are interpreted together.

When testosterone therapy is medically appropriate, the formulation, dose, route, and frequency are individualized. Smaller, more frequent injections may be considered for some patients to reduce large peak-and-trough fluctuations associated with injectable testosterone.

Fertility and testicular function

Exogenous testosterone can suppress LH and FSH and reduce intratesticular testosterone and sperm production. For men who want to maintain fertility or testicular function, treatment options may include avoiding testosterone, using medications such as clomiphene, enclomiphene, or hCG when appropriate, or obtaining a semen analysis not provided by Vitali-T. No medication guarantees preservation of fertility.

Enclomiphene

Enclomiphene is a selective estrogen receptor modulator that may increase LH and FSH and stimulate endogenous testosterone production. Clinical studies have shown increases in testosterone while maintaining gonadotropin signaling and sperm concentrations in selected men. There is currently no FDA-approved finished enclomiphene drug product in the United States. When prescribed, it is generally provided as a compounded medication subject to applicable federal and state requirements.

Clomiphene

Clomiphene is an FDA-approved medication, but its use in men for testosterone deficiency or male infertility is generally off-label. Off-label prescribing of an FDA-approved medication is not inherently unlawful when an appropriately licensed practitioner determines that the treatment is medically appropriate and complies with applicable laws and professional standards.

hCG

Human chorionic gonadotropin, or hCG, acts similarly to LH and stimulates testosterone production within the testes. FDA-approved hCG products have selected hormonal and reproductive indications. Its use alongside testosterone to help support intratesticular testosterone, testicular function, or fertility is generally considered off-label.

Women’s hormone optimization

Vitali-T also evaluates women experiencing symptoms associated with perimenopause, menopause, surgical menopause, sexual-health concerns, or other hormone-related changes. Treatment may include estrogen, progesterone, localized vaginal therapies, testosterone in selected patients, or nonhormonal treatment. Therapy is individualized based on symptoms, laboratory findings when clinically useful, uterine status, medical history, risk factors, treatment goals, and response.

Medical weight management

Vitali-T provides medically supervised weight-management care for eligible men and women. Programs may include:

• Medical evaluation
• Metabolic testing
• Nutrition guidance - coming soon
• Resistance training and activity recommendations
• Body-composition monitoring - self reported by patient
• Prescription medication when appropriate
• Ongoing safety monitoring Medication may support weight loss but does not replace nutrition, exercise, adequate protein intake, sleep, and long-term lifestyle change.

Performance medicine

Performance medicine at Vitali-T focuses on health factors that influence function, recovery, body composition, energy, strength, metabolic health, sexual wellness, and healthy aging.

Performance medicine is not limited to competitive athletes and does not mean prescribing medications solely for bodybuilding or athletic enhancement. Selected adjunctive therapies may be considered when there is a legitimate clinical rationale, sufficient evidence, an appropriate legal and pharmacy pathway, informed consent, and an appropriate monitoring plan.

Medication transparency

Vitali-T may use medications with different regulatory classifications. FDA-approved use means an FDA-approved medication is being used according to its approved labeling. Off-label use means an FDA-approved medication is prescribed for a different indication, population, dose, route, or schedule. Off-label prescribing is common in medicine and may be appropriate when supported by clinical judgment and applicable law. Compounded medications are prepared by appropriately authorized pharmacies for individual clinical needs. Compounded medications are not FDA-approved and are not reviewed by FDA in the same manner as approved finished drug products. Investigational medications are different from both off-label and compounded medications. Vitali-T does not represent investigational medications as ordinary off-label treatments.

Telemedicine care

Vitali-T currently provides telemedicine care to eligible patients located in Oregon and Washington. The treating practitioner must be licensed or otherwise legally authorized to provide care in the state where the patient is physically located. Current federal DEA and HHS telemedicine flexibilities allow qualifying DEA-registered practitioners to prescribe Schedule II through V controlled substances, including Schedule III testosterone, through appropriate audio-video telemedicine encounters without first completing an in-person medical examination. These federal flexibilities currently remain in effect through December 31, 2026. Telemedicine does not remove the practitioner’s responsibility to:

• Conduct an adequate medical evaluation
• Establish medical necessity
• Review appropriate laboratory information
• Identify contraindications and risks
• Monitor treatment
• Maintain appropriate documentation
• Arrange an in-person examination when clinically necessary
• Follow applicable state and federal law ## Current and future service areas

Vitali-T currently serves:

• Oregon
• Washington The clinic plans to expand first into additional western states and then gradually into more states throughout the United States. A state should not be considered an active Vitali-T service area until the clinic has established the required practitioner licensure, prescribing authority, pharmacy access, and clinical systems necessary to provide care there.

What Vitali-T is not

Vitali-T should not be described as:

• A nationwide clinic at this time
• A remote-only practice
• A retail pharmacy
• An automated prescription service
• A substitute for primary care
• A clinic that guarantees medication
• A clinic that uses the same dose for every patient
• A bodybuilding-cycle service
